A Fatal 5 Audit is performed a minimum of 2-3 times per month per project. They are
performed in Procore and are shared with the team.
Each audit is performed consistently using categories based on our scope of work and
at the same time other potential hazards from other trades.
Wehn an audit is performed the Project Leader signs off, any item found is gone over
with the Project Leader to ensure that they understand the reason it is
unacceptable.
Each Project Leader, Crew Leader, and Safety Representative are required to complete
a weekly Safety Observation. This allows for the Safety Team to develop training and
Toolbox Talks based on data collected from the Safety
Observations and Safety Audits.
Safety Audits and Safety Observations are discussed in weekly meetings that includes
OPS management, Field Leadership and Safety Manager.
